Park District Forms

Registration forms may be dropped off at any Park District facility, mailed to the Administrative Offices at 1200 Wilmette Avenue, Wilmette, IL 60091 or faxed to the Park District's Registration Fax Line at 847-256-0739. Faxed registration forms MUST be accompanied by credit card information.
